Library Equipment Agreement Form: Home

Filling out this form is a prerequisite for checking out equipment from the MATC Libraries

The process for checking out Chromebooks, Hotspots, and other technology is changing from 'first come, first served' to an application process. The Application for Technology Assistance is currently closed, but will reopen for Winterim and Spring classes by 12/16/24. If you have questions about an application you already filled out, fill out this form. If approved, you will be notified of the pickup date in your MATC email. You can also sign up for text message alerts.

Chromebook and Hotspot Info

MATC Libraries’ limited supply of Chromebooks and Hotspots available for checkout is changing from 'first come, first served' to an application process. The application is currently closed, but will reopen by 12/16/2024 for the Winterim and Spring semesters. Fill out this form, if you have questions about an application you already filled out. 

When the form re-opens in December, you must fill out the application to be considered; equipment is not guaranteed. If approved, you will be notified in your MATC email. You must pick up the device by the specified date in the email, otherwise the equipment will be assigned to another student and you will be placed back on the waitlist. You can also sign up for Text Message Alerts and get notifications on your mobile phone as well. The approval process can take up to 5 business days or longer during peak periods.

 

Step 1: Fill out the Application for Technology Assistance now.

Step 2: Fill out the Library Equipment Agreement Form prior to pick up.

Step 3: You can also sign up for Text Message Alerts, and get notification on your mobile phone.

 

  • A Stormer Pass/MATC ID is required for checkout.
  • Devices must be returned by the due date.
  • Devices may not be renewed and must be returned for maintenance before they are available for checkout again.
  • Overdue late fees and replacement charges will be applied if devices are not returned by the due date, and all devices will automatically be deactivated.
  • Please return them to the Libraries' Circulation Desk and NOT in an outside drop box as this may cause damage.

  • The Downtown Milwaukee Campus Library has a limited supply of laptops and devices for Animation and CSG students.
  • Macbooks are also available to Graphic Design (GRDS) or Audio Production (Audio) students.
  • Students must show class schedule indicating ANIM, CSG, GRDS, or Audio enrollment. Please call ahead for availability,        414-297-7030.
  • Please fill out the Library Equipment Agreement Form prior to checkout.

Replacement costs for equipment

You will be responsible for items that are lost, damaged, or stolen, while they are checked out in your name, so please don't leave equipment and/or devices unattended. Replacement costs for equipment are as follows, and are subject to change:

  • Chromebook cords: $15.00
  • Chromebooks: $400.00
  • Chromebooks with wifi : $675.00
  • Chromebook case: $50.00
  • Laptops (IT):  $1,100.00
  • Laptops (Library owned): $1,900.00
  • Laptops (Animation): $2113.00 - $2,538.00
  • Laptops (Accounting): $1,044.00                   
  • Laptop cords: $15.00
  • Hotspots:  $360.00
  • Hotspot cords: $15
  • Hotspot Laminated card: $5.00
  • Macbooks:  $2,199.00
  • Leap Motion Controllers:  $105.00
  • Tablets (Animation):  $90.00
  • WaveLab software key: $280.00
  • Wacom tablets: $380.00
  • Other item replacement charge: Cost of item or $100, whichever is higher
  • Damage charge: $75.00 up to replacement charge
